Vermont Blueprint for Health Web Portal
Stone worked with the Vermont Blueprint for Health to replace their spreadsheet system for tracking approximately 121 Blueprint recognized practices, 738 healthcare providers, and 270 Community Health Team staff in 14 Health Service Areas across Vermont.
